This is the official Constitution of Valleybrook Community Church
The attached is an overview of the qualifications of Elders as set forth in 1 Timothy 3:1-7 and in Titus 1:5-9 along with Valleybrook Community Church’s additional qualifications.
The process of elder recommendation through the congregation has been developed at Valleybrook Community Church to allow members of the Church a voice in recommending and motivating qualified servant leaders to guide God’s work within our Church family. This form is a detailed overview of the entire selection process.
MISSIONS AND FUNDRAISING
Individuals taking short term and mini missions trips
The purpose of this policy is to create an organized framework in which Valleybrook can encourage and help facilitate the active, personal involvement of the congregation in kingdom work around the globe; additionally, it will clarify support-raising procedures within the church for various types of trips.

You may request copies of past sermons in CD format. Please leave this form at the Welcome Table. Many titles are listed under our SERMONS tab and there is also a catalog at the Welcome Table.
The Children’s and Youth ministry of Valleybrook Community Church shall be implemented in a safe and secure manner. Staff, volunteers and parents shall follow procedures governing; supervision of children, screening and qualifications for staff and volunteers, incident reporting, illness, medications, allergies, discipline, restroom, privacy, sanitation, photo publication and injury response. Ministry leaders shall train ministry team members in these procedures and give an annual report of the training as part of the annual ministry report in August.
MINISTRY APPLICATION FOR CHILDREN/YOUTH WORK
Application and Statement of Faith for Volunteers
This form must be filled out, signed and dated then given to The Children and Youth Director along with the Volunteer Release and Waiver if you are interested in working with the youth of Valleybrook.
VOLUNTEER MANDATED REPORTERS
This form is to be filled out by all staff /volunteers working with minors
The State of Illinois has a mandatory duty to report known or reasonably suspected cases of child abuse. These mandated reporters would include clergy, teachers, coaches, childcare workers, youth workers and any other person with responsibility for care of children. The State of Illinois has enacted child abuse reporting statutes in an effort to protect abused children and prevent future abuse.
VOLUNTEER RELEASE AND WAIVER
Criminal Background Check
This Criminal Background Check Release and Waiver must be filled out and submitted to Valleybrook Community Church staff prior to any volunteer working with minors.

This form must be signed and dated by parents/guardians of each participant for off campus youth activities.
GUARDIAN CONSENT FORM
Permission Slip for Church-Approved Transportation
Please fill out this form when your child will be using Church-approved transportation for an event.
HEALTH & EMERGENCY CONTACT FORM
Please fill out this form when your child will be participating in any Church-sponsored Trip or Missions Trip.
